Things to Bring to Your Appointment:

• A list of your current medications along with your prescription bottles with medicine.

• Radiology images (including paper radiology reports) supplied by your local imaging center and/or physician.

• The name, address, and telephone number of the physician who referred you, if applicable.

• Your insurance card and Medicare card if you have one.

• Referral number or authorization (if applicable).

• All recent lab tests, medical records, and pertinent X-rays.

• Copay. Deductible, or out of pocket expenses.

• All forms can be mailed to you or you may print out the forms at the top of the page, fill them out, and bring them with you.

Authorization to Release Information Form

Your registration materials include a form that authorizes The Movement Disorder Clinic of Oklahoma to release information to your insurance company. Accurate and complete authorization forms help assure that your claims will be promptly paid. Please complete a new Authorization to Release Information any time your coverage changes.

Insurance Coverage

Your insurance may, or may not, cover the services provided at the Movement Disorder Clinic of Oklahoma. To obtain the most accurate information, please check with your insurance carrier to discuss the benefits provided by your medical plan prior to your visit.

Payment Forms Accepted

You are responsible for payment of your clinic accounts. Your co-payment or portion of your deductible is due at the time of service. For your convenience, The Movement Disorder Clinic of Oklahoma accepts cash; personal checks; Visa and Master Card credit cards.
